Hampshire students: Please click "Sign Up" at the bottom of this page, create a username and use your official Hampshire email, (i.e. xxx22@hampshire.edu), then click "Submit." You will receive an email instructing you to create a password.

Other UHS patients: Please click "Sign Up" at the bottom of this page, create a username and use the email on file at UHS, then click "Submit." You will receive an email instructing you to create a password.
